• Blood glucose monitoring and how to use the results
• Controlling high and low blood glucose
• Proper use of diabetes medications
• Proper nutrition-meal planning, portion control and reading labels
• Preventing diabetes-related complications
• Exercise and diabetes management
• Healthy practices for skin and foot care
• Daily living concerns and family involvement
• The benefits of committing to your own health care

An individual assessment of your learning goals is completed as part of your education program. Individual appointments for meal planning or additional information are available. Ongoing follow up and support is provided.
