A referral from your health care provider is necessary for registration for Providence Diabetes Center programs. Please ask your health care provider for a referral or call 937-396-8137 for more information. The Providence Diabetes Center offers comprehensive diabetes education sessions in several different locations including Huber Heights, Beavercreek, Dayton, and Germantown. Individual appointments with the dietitian and nurse educator are also available.
