Family Care Center offers personalized primary care for patients of all ages, and has served families in the Dayton community for over 25 years. In addition, Family Care Center provides
- Specialization in the treatment of depression, bipolar disorder, diabetes and high blood pressure.
- Acute and ongoing patient care from Certified Family Nurse Practitioners Jacquelin Davis and Mary Sroga, MS, CRNP. Mary also specializes in diabetes management and diabetes education.
Board Certified in Family Medicine, Carl F. Hoyng, DO, is a graduate of the West Virginia School of Osteopathic Medicine. He also completed an internship in Medical Technology at Christ Hospital in Cincinnati, Ohio. He was board certified as an MT (ASCP) by the American Society of Clinical Pathologists, and has earned an MS degree in Medical Microbiology from the University of Dayton. Dr. Hoyng maintains a special interest in Laboratory Medicine and is currently Medical Director of the Providence Medical Laboratory.
Paul A. Martin, DO, FACOFP, is Board Certified in Family Medicine. He has been a Fellow in the American College of Osteopathic Family Physicians since 1997, and a member of the Board of Governors of the ACOFP since 2002. Recently, Dr. Martin was elected President of the ACOFP; he is a Past President of the Ohio Chapter of the ACOFP, and a Past President of the Ohio Osteopathic Association. Dr. Martin has also served as Chief of Staff of Dayton’s Grandview/Southview Medical Center, and is a past member of the Board of Governors for the Chicago College of Osteopathic Medicine, and the Board of Trustees of Midwestern University.

This voluntary program is designed to recognize physicians, who use evidence-based measures and provide excellent care to their patients with diabetes.
