Kevin A. Carter, DO, FAASM is Board Certified in both Family Practice and Sleep Medicine; he was recently awarded the degree of Fellow by the American Academy of Sleep Medicine.

Dr. Carter recently served as Medical Director at the United States Army Sleep Medicine Center at Fort Benning, GA. He is a graduate of the Ohio University of College of Osteopathic Medicine.

Dr. Kevin Carter provides full-spectrum sleep medicine evaluations, diagnosis, and treatments at the Carter Sleep Center, located in the offices of Beavercreek Commons Family Practice. The Center treats sleep-related breathing disorders (Obstructive Sleep Apnea), insomnia, narcolepsy, restless leg syndrome, and parasomnias in all age groups. Steven Folkerth, MD, is board certified in Internal Medicine, and offers personalized, compassionate care with over thirty-five years of experience in adult health and wellness. He is also a certified Clinical Trial Investigator, and oversees a range of clinical trials for the Providence Health Partners Center for Clinical Research.

A native of Dayton, Dr. Folkerth received his medical degree from the University of Cincinnati College of Medicine, and performed both his internship and residency at the Kettering Medical Center.

Dr. Ryan Bayko, board certified in family medicine, has joined Beavercreek Commons Family Practice’s professional provider staff, and is accepting new patients.

Dr. Bayko received his medical degree from the University of Cincinnati College of Medicine, and has been serving southwest Ohio families for over ten years. Professional honors include Wright State College of Medicine Preceptor of the Year and Wright State Family Medicine Preceptor of the Year.

However, if you elect to seek your care from another provider, you should take some important steps to ensure the continuity of your care and to comply with medical records regulations:

  1. Check with your health insurance carrier that the new provider you have selected is covered by your particular insurance plan.
  2. Fill out an Authorization to Transfer Medical Records form to have your records properly transferred to the new practice.
